Social Prescribing
This short animation explains what it social prescribing is, how it works and the benefits to individuals' health and wellbeing.
A team of Social Prescribers are working alongside the GP surgeries and helping the patients in dealing with different social issues.
How it helps
Social prescribing can help individuals to have more control over their own health and find ways to improve how they feel in a way that suits them. Social Prescribers listen to the patients, put them in touch with the people, organisations and activities that aim to help them to feel better. They might help to find legal advice, debt counselling, digital resources, community group, a new activity and much more. The service is free and confidential. Please ask reception team for more information. Referrals are made via GP surgery.
